How your health records are shared

We have recently been advised that NHS England will be extracting patient data from our clinical system to help link the hospital statistic data they already hold. This data will be collected automatically from our system on a monthly basis (including patient identifiable data from April 2013 onwards).

By law we have to comply with this although patients do have the right to opt out of having their data extracted. You can object to your personal confidential data leaving the GP practice and you can also prevent personal confidential data gathered from any health and social care setting from leaving the HSCIC (Health and Social Care Information Centre). We are trying to contact all of our patients to let them know about this and give them the opportunity to opt out, if they wish to do so.

Further information can be found
here . You do not need to take any action, unless you wish to opt out of this information extraction. If you wish to opt out, it is important that you let us know as soon as possible (by 24th October 2013). You can let us know in person or by writing to us or you can email us at   If in the past you chose not to have a summary care record created, you will automatically be opted out of this data extraction. Should you wish to change this, please contact us.

Please note that if you have responded with instructions to opt out of this, we will action. However we are unable to send out confirmation emails.
